Help maximize the time patients and families have together as a Social Worker (Nonexempt) at AseraCare by performing a variety of daily tasks.

Provides medical social services to the patient and family in accordance with the interdisciplinary plan of care so that the function of each patient and the coping capacity of each family member will be maximized.

Essential Job Functions

  • Provide medical social services under the direction of a physician. Assists the physician and other members of the hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health problems to establish a plan of care that fosters the personal worth, spiritual well-being, and human dignity of each patient.

  • Participates as a member of the Interdisciplinary Group and actively participates in the development and review of the plan of care for assigned patients

  • Assesses the social, spiritual, and emotional needs/factors in order to estimate the patient's and family's capacity and potential to cope with the problems of daily living and with the terminal illness

  • Helps the patient and family to understand, accept and follow medical recommendations and provide services planned to assist the patient and family in achieving the optimum social, spiritual, and health adjustment within their capacity.

  • Prepares the patient to deal with changes and the family to support the patient. Observes, records, and reports social, spiritual, and emotional changes

  • Utilizes all available resources, such as family, hospice, Master's prepared social workers and community agencies, to assist the patient and family to live better within the limitations of the illness

  • Documents all services in the patient's clinical record

  • Participates in ongoing educational programs

  • Contributes to team effort by accomplishing related results as needed. From time to time this position will be expected to perform on-call professional duties

Qualifications

  • Bachelors Degree from a school of social work accredited by the Council on Social Work Education
  • Minimum two (2) years social work experience in a health care delivery system preferred
  • CPR certified
  • Must have valid driver s license, 24-hour access to a motor vehicle and maintain personal auto liability insurance coverage
  • Must be capable of maintaining regular attendance
